We are pleased to inform you that we offer our Functional Medicine and Medical Weight Loss services in the convenience of your home. In order to ensure that we have all the proper documents we ask that you follow our simple 3 step approach. A copy of your insurance card and ID is also needed and it can be uploaded as well. We thank you for choosing us for your personalized health needs virtually.

How to Prepare for Your Telehealth Consult?

  1. Access and Fill Out Forms:
    • Please complete the forms below that we have advised you to fill out. All NEW PATIENTS must fill out all Initial Intake Forms.
    • Please upload your State ID Card or Driver License.
  2. Upload Past Medical Records and Bloodwork:
    • Please upload any recent blood work within a year and your last physical exam records.
    • Ensure all information is accurate and up-to-date to facilitate a comprehensive assessment.
  3. Pay Deposit and Schedule Appointment:
    • Once our team receives all your forms, expect a call from one of our team members to schedule your telehealth appointment with our Nurse Practitioner, Bertha, if you do not already have one.
    • Please be advised that a $50 deposit is required to confirm your appointment and all paperwork must be received prior to your appointment or it will be canceled.
